Please be aware of the following information when making your records request:

MetalQuest does not store all of the records for any of these hospitals. Each request form tells you those records we store.

The information we ask for is necessary to be incompliance with patient confidentiality and the applicable laws. It also helps us find what you need. We ask for phone contact information because we sometimes need information to clarify the request. We also need the signed form even if we are simply verifying that the information you need is included in the records we have; this is part of the Federal HIPAA regulations.

The request form must be signed by the patient in question or a legal caregiver (power of attorney, medical care provider).
